You might not like filters but Noise Ninja is a great PS plug for removing noise. Just use it in moderation and add a little unsharp mask to it after you are done.

For anyone interested:
What I do is open the image, click "filter" then select "picture code" then "noise ninja". When it opens click "profile image" and let it do it's thing. When it's done click "filter" from inside noise ninja and work with the siders on the left hand side making sure you do not burn out anything. After you use the noise filter click inside PS "image" and then "mode" and change it to "lab color". In your PS layers box click "channels" and click "lightness". Now just go to filter and then unsharp mask and enter these average settings.

threshold 4
When done click "image" and then "mode" and change it back to RGB color. Now save it.

but IMHO, a little exaggerated use of filters
maybe i'm wrong... but the subject look a little unreal....
