Once I had all of the rest of the image how I wanted I flattened it. Then I duplicated that layer and went up to gaussian blur and slid the arrow over until it was really pretty blurred. Then I put an overlay mask on the blurred layer and painted through what I wanted sharper. I did it with a brush on a low opacity so I could build it in slowly until it looked just like I wanted it to and then reflattened it all. :) Hope that makes sense... it's pretty easy and clients LOVE it.
